Receiving Services

FAQ: I have a disability. How do I obtain a service from the Center for Human Services?

We have a variety of services of which each service has a different eligibility requirement. For individuals that are age 3 and older, please call our Intake Coordinator, Sonia Setzer at 660-826-4400, ext#398. For children that are developmentally delayed that are younger than 3, please call our First Steps SPOE Intake Coordinator, Connie Dove at 660-827-5004, ext#236.


FAQ: Can anyone with a disability receive services from the Center for Human Services?

No, not everyone with a disability will qualify for our services. Many of our services are funded by state agencies and these agencies tell us what criteria to use in determining if someone is eligible for certain services.


FAQ: If I am an adult with a disability, how do I know if I am eligible or not for a service?

You can contact the Adult Services Intake Coordinator, Sonia Setzer at 660-826-4400, ext#398 and she will help determine what service you are interested in and can explain the eligibility determination process. For all CHS services, other than First Steps and Early Head Start, we have an Admission, Placement and Suspension Committee that makes final determination regarding eligibility into our programs. Our CHS Utilization Review Committee makes determination of available funds and approval to use those funds for requested services. Once your referral/request for service has been through our Admission's and Utilization Committee, then you will be notified in writing of the eligibility/funding decision.


FAQ: Is there a waiting list to be employed at the Cooperative Workshop?

Yes


FAQ: How long is the Workshop waiting list?

The best way to answer this is to explain that the reason the Workshop has a waiting list is that it has been difficult for the Workshop to get new job contracts. Therefore, when there isn't any new work to create job openings, then we aren't able to hire individuals in the Workshop. We have a wonderful marketing department that is always pursuing jobs, but it is unknown when the Workshop will get more jobs in and when we will then need to hire more people. Also, when a job does come open in the Workshop, we don't just automatically hire the person on the waiting list who has been waiting the longest. We first determine what skills are needed to do the job and then we hire the best person from the waiting list who we have assessed is the most capable of doing the job. We match the skills required for the jobs to the people that have these skills. In summary, once an individual has been placed on the Workshop waiting list, we do not know how long the person will have to wait in order to be hired at the Workshop.


FAQ: Do you have a Sheltered Workshop in Benton Co?

No we do not. The Employment Services Day Program in Benton County is operated as a Day Habilitation Program for the Department of Mental Health Developmental Disability Division. The eligibility criterion is restricted to individuals who are enrolled in the DMH Developmental Disability Division and must also be eligible for the DD's Division Medicaid Waiver Program. The only Sheltered Workshops that we operate are located in Pettis and Saline Counties. Sheltered Workshops are Employment Programs that are overseen by the Missouri Department of Education.


FAQ: Is a Consumer Advocate Services Professional and Case Manager the same thing?

Yes, our Case Managers used to be called Service Coordinators and now their title is Consumer Advocate Service Professional. You can shorten their title to Consumer Advocate or CASP.


FAQ: How can a Consumer Advocate Service Professional (CASP) help me?

If you have been found eligible for the Department of Mental Health's Developmental Disability Division by the Central Missouri Regional Office or if you have been hired to work at the Workshop, you will be assigned a CASP. Your CASP will help coordinate services for you so you can receive the support and services that you need. Your CASP will help you identify what your goals are and what services/supports you need to accomplish your goals. Your CASP will then help you develop a plan to obtain those services. Your CASP will locate services for you, provide information to you, make referrals on your behalf, advocate for you and monitor the services you receive to assure that you are satisfied with your services. Your CASP will not provide direct services to you such as transporting you places, providing therapy to your or taking care of your physical personal care needs. Your CASP will help in finding services to provide assistance to you for these types of needs.

FAQ: What are the services that Community Living offers?

Training and habilitation, community support coordination, community inclusionary activities, recreational opportunities, social events group, affordable housing , community RN program consultation


FAQ: What are the entrance criteria for the Activity Center?

Must have a developmental or other identified disability; a payment source identified; and following a review of a functional assessment and identified training needs, receive approval of Admissions/Placement/Suspension
